National Health Insurance

Conditions to apply

The National Health Insurance is a system providing public medical assistance for people who did not join to any health insurance scheme. Foreigners who registered their address at the City Hall, excluded people whose status of residence falls in the category of Designated Activities (i.e. medical treatment), and all the people who did not join to any other insurance scheme must enroll to the National Health Insurance.
Depending on the purpose of their stay, foreigners who did not register their address to the City Residents Registry may also be eligible to apply to the National Health Insurance if they plan to stay in Japan for more than 3 months. Please send us your inquiries for more information.

Required Documents
1.  Special Permanent Resident Certificate or Resident Card (in case you do not have any of these documents, your passport can also be accepted).
2.  Applicants who previously joined to another insurance scheme must bring a certificate proving that they already canceled the previous insurance scheme.
3.  Foreigners who did not register their address to the City Residents Registry must bring a document stating their status of residence, purpose and expected period of stay (this must be longer than 3 months).


National Health Insurance Card

When you join to the National Health Insurance, you will receive a National Health Insurance card. Please remember to always bring this card with you when you visit a hospital or a clinic, because you have to show it to the personnel at the counter. In case your name, your address or any other information entered in the card change, please report those changes to the City Hall officers within 14 days. Also, please be reminded that you will have to return the National Health Insurance card when you move back to your country or when you apply for a different health insurance scheme.

National Health Insurance Tax

How tax is calculated
The tax will be calculated for single households in accordance to insured memberfs income and household size and situation during the previous year. All members who are from 40 to 65 years old must also pay the long-term nursing care contribution in addition to National Health Insurance tax.
Tax reductions may be applicable to those members whose income in the previous year was lower than the designated amount (people who did not have any income and people who only had non-taxable income are also included).

How to pay the tax
The tax is divided into 8 monthly installments to be paid from July to February. You can pay the installments at many places, such as banks and convenience stores, using the Statement of Payment (noufusho) that will be sent to you by mail from the City Hall.
If you wish to pay through bank transfer, please proceed with the due special procedures at your banking institution.

(Attention)
If you let the National Health Insurance tax fall into arrears, you may be charged to pay the whole amount of the medical fees after receiving services from hospitals or clinics.

Insurance benefits

In case of illnesses or injuries, showing your insurance card to medical institutions authorized to treat patients in possess of health insurance coverage will allow you to pay only the 30% of the medical fees. The remaining 70% will be paid by the city of Maebashi. In case of childbirth, households are granted a fixed allowance called
gOne-Off Payment for Childbirthh (shussan ichijikin). Also, in case the insured member dies, the person who will hold the funeral can receive a fixed allowance for the funeral expenses.
