Family Registration

Registration of new births, deaths, etc.
The Family Registration Law regulates changes of individuals' status or social position and is applied to all Japanese and foreign nationals who reside in Japan. Therefore, as established by the Family Registration Law, in case a foreign national is born or dies in Japan, it is necessary to report their birth or their death to the local municipal office.


Registration of new births and deaths

Birth Notice

Deadline
Any new birth must be reported within 14 days.

Where and how to report
The new birth must be notified to the municipal office of the city, ward, town or village, where the child was born or the reporting person lives.

Required documents
1.  Birth Registration Form (Shusshou Todokedesho)
It comes with the Birth Certificate issued by the hospital.
2.  Maternal and Child Health Handbook (Boshi Kenkou Techo)
  Expectant mothers can receive this handbook at the Healthcare Center (Hoken Center) in the city, town, ward or village they reside when they submit the Notice of Pregnancy
(Ninshin Todoke).
3.  National Health Insurance Card (Kokumin Kenkou Hokensho), or Social Health Insurance Card (Shakai Kenkou Hoken) issued at your workplace.


Death Notice

Deadline
Any death must be reported within 7 days.

Where and how to report
Any death must be notified to the municipal office of the city, ward, town or village, where the dimised person was living and residing.

Required documents
1.  Death Registration Form (Shibou Todokedesho)
It comes with the Death Certificate issued by the hospital.
2.  National Health Insurance Card (Kokumin Kenkou Hokensho)
  Only in cases when the demised person registered to the National Health Insurance system.


New Family Relationship Establishment Report

As established by the Family Registration Law, in case an individual marries with a foreign national or a Japanese national, or adopts a child, as well as in other cases when he establishes new family relationships, he/she must visit the local municipal office and submit a notice to report the fact (Marriage Notice, Adoption Notice, etc.).


Acceptance of Notification

In case you need a certificate to prove you have registered a birth, a death, a marriage or an adoption, the municipal office where you made the registration will issue a certificate called Acceptance of Notification (Todokede Juri Shoumeisho).
